If you’ve ever experienced acne, you know that once a pimple finally clears, the skin often looks red or irritated in the area where the original spot was. There is a technical term for this: post-inflammatory erythema, often referred to as PIE. And it can often be just as stubborn and frustrating as the original acne itself.
“When inflammation occurs associated with an acne breakout, it can cause increased and dilated blood vessels and vascularization,” explains dermatologist Marisa Garshick, MD. “What can give the skin a red appearance, related to an increased concentration of blood vessels in that area. Also, the general redness that goes with inflammation can persist and also contribute to post-inflammatory erythema. “PIE, or post-inflammatory erythema, refers to pink or red spots that occur as a result of trauma that causes inflammation in the skin,” says Riyaz. “This is usually due to inflammatory acne. Mild, non-inflammatory acne that includes blackheads and whiteheads, or comedonal acne, is not usually a cause of PIE. Acne with inflamed papules and pustules is more likely to cause PIE. Papules are red inflamed bumps which are usually less than 5 mm or a tenth of an inch, with pustules with a white or yellow center.

It feeds on excess oil and causes inflammation, redness and infection that manifests as acne. When bacteria digest the trapped oil, it produces fatty acid residues that cause irritation. This exact irritation can translate into PIE, which often leads to pink or red patches on the skin.

If you’ve ever used a harsh product that didn’t agree with your skin, chances are you’ve dealt with irritation and redness as a result. “While there are many acne treatments, it’s important not to overdo it and avoid harsh soaps and abrasive scrubs, which can irritate the skin and worsen redness and post-inflammatory erythema,” says Garshick. “Avoid using too many active ingredients at once to break the skin barrier and cause excessive irritation.”
“Plucking, picking, or squeezing acne can increase your risk of PIE because it can lead to more inflammation, which can later lead to more redness and scarring,” says Garshick. This is one of the main reasons why it is best to avoid thorns, no matter how tempting they are.
