The revised protocols that will come into effect on October 11 “mark a shift in emphasis towards personal responsibility and self-management”, MOH said.
The protocols will change for those who do not survive and also test positive for COVID-19 – they will be diagnosed using an antigen rapid test (ART) instead of the polymerase chain reaction (PCR) test that is currently the default. They will also be released from isolation based on the number of days they have been in isolation rather than a negative test.
Those who are well and positive will also not need a PCR test, and will need to remain in isolation until their tests show a positive reading after 72 hours.
In a press conference by the multi-ministerial COVID-19 task force, Health Minister Ong Ye Kung said the Delta variant, which is more contagious and has higher viral loads than previous variants, has changed the country’s risk calculation.

“ART, including self-swabs, does a pretty good job of detecting someone who is infected and infectious, making ART more useful than ever in detecting COVID-19 patients,” he said .
The changes come as authorities push a “reset button” in a “huge effort” to simplify rules that have accumulated over 20 months, MOH said.
“Some people tell me that they are more afraid of these rules than of COVID-19 itself and this is a feeling and a problem that needs to be addressed,” said Mr Ong.
Close contacts of COVID-19 patients will be given health risk warnings, while moving away from “long-term quarantine orders”, MOH said. They will have to do ART on the first day and upload the results on a portal, MOH said.

In the following days, if they are going to go out, they must test negative with ART on the same day. They do not need to test beyond the seventh day, as the period of greatest concern for infection has passed, MOH said.
“Previously, we differentiated contacts of COVID-19 cases into different levels of risk and applied different measures,” she said. These measures included quarantine orders, health risk warnings and health risk warnings.
“Now there will be one approach – a health risk warning that will last seven days from the day of its issue, which is based on self-testing ART.”
Mr Ong said ART will take most of the close contacts who have already tested positive. Repeated ART testing can detect most of the rest, he said.

Those receiving health risk warnings can collect ART kits from vending machines, the number of which has doubled from 100 to 200, MOH said. Each recipient of the health risk warning will receive six test kits. A total of 7.5 million devices were allocated for this purpose.
If the doctor assesses the possibility of having a COVID-19 infection, then the person will be tested, MOH said.

The majority will go through ART without the need for a PCR test and if they are found to be positive for COVID-19, the patients will have to go home and begin their isolation. The doctor notifies the MOH, who then contacts the patient for further instructions on what to do.
Those who are fully vaccinated will need to self-isolate for 10 days and those who are not vaccinated will need to self-isolate for 14 days. The MOH said they have “relaxed” the requirement for self-isolators to have a toilet and ensuite bathroom in the same room.
This applies as long as people can clean the bathroom after using it so that other members of the family are not exposed to the infection.
They are automatically placed on the home recovery program, or if the home environment is not suitable, they can be recovered in appropriate care facilities, MOH said.

MOH representatives will call patients to ask them questions about who lives in their home and their home circumstances.
This will provide a better understanding of the situation so that advice can be given on whether it is safe to continue isolating at home, or whether arrangements should be made to provide accommodation in a community isolation facility, MOH said.
At the end of their isolation, their TraceTogether phone application will automatically update that they are “released”.
Those who are well and undergo ART for reasons such as pre-event tests, will have to self-isolate for 72 hours and take another test at the end of that time.

Despite having to be isolated, these people can still make a trip to a vending machine to get free ART kits, MOH said.
If they are found to be free of COVID-19, people can end their isolation and resume their normal activities. Until the test shows a positive reading, however, the person should self-isolate, MOH said.
If a person has symptoms such as high fever or shortness of breath, the person should see a doctor, MOH said, adding that telemedicine services may be an option.
Employees who have been diagnosed with COVID-19 through ART and are “physically” may work from home if possible. If it is not possible to work from home, they will have to take paid sick leave, but they will not have to produce a medical certificate, MOH said.
